Supportive Housing Case Management

The goal of the Supportive Housing Service is to help people with mental health problems who are homeless or at risk of becoming homeless to gain access to affordable housing and, to provide them and their families with the required supports to enable these individuals to live in the community. Stable accommodation has been identified as a key factor for consumers on their way to recovery.
This Service works on a 'head lease' model with Case Management support to allow consumers to live in public market units. Hong Fook Mental Health Association rents the apartment units or house units in the private market for their consumers then provides them with services such as:


  1. Problem solving.
  2. Crisis management.
  3. Coping with emotions.
  4. Managing mental health problems.
  5. Linking to community resources.
  6. Developing positive relations.
  7. Life skills training.

Who can apply? Individuals who:

  1. Are homeless or at high risk of becoming homeless.
  2. Have a mental health problem.
  3. Are 16 years of age and over.
  4. Have an income or eligible to receive an income.
  5. Are ready and able to live independently.
  6. Have linguistic and cultural barriers in accessing other mental health services.
