Hong Fook Mental Health Association is a community organization serving the Greater Toronto area. We provide leadership in promoting the mental health of people in the Cambodian, Chinese, Korean and Vietnamese communities. Hong Fook helps people keep mentally healthy and manage mental illness. We achieve our mission through service, education, advocacy and research in a supportive environment.

Title: |
Mental Health Worker - Full / Part time (1
year contract)
Hong Fook Mental Health Association is seeking
an individual who has extensive clinical skills
and knowledge, enjoys working in a team
environment serving clients with severe and
persistent mental illness from the Vietnamese
Speaking Community.
|

|
|
Job Responsibilities: |
• Support client’s recovery through the provision of intensive case management, group work and self-help programs
• Provides a range of service/support including: client-centred assessments, treatment, rehabilitation and support services.
• Works with families to promote mutual support, capacity building and empowerment.
• Offers support to the following programs/ services as needed: Intake/Consultation; Consumers Self-Help Initiatives; Case Management in Supportive Housing; Health Promotion: System Advocacy
• Maintains effective and timely documentation in client records
• Identify and address barriers that impact on the
Vietnamese community’s ability to access mental health services
• Participates in providing education to community service providers and other stakeholders regarding
Vietnamese cultural diversity and awareness
• Provides staff back up for any agency programs
• Completes other duties as assigned

Working Conditions: |
• 37.5 hours per week
• Office Work, Community settings, meetings at client homes and travel with clients
• Flexible schedule: including some evening and weekend
|
|
|
|
Skills and Qualifications: |
• BSW preferred; University degree or college diploma in Social Services or Human Services related field accepted
• Current registration with OCSWSSW or is eligible for the OCSWSSW membership
• At least 2 years of case management experience and /or front line experience with vulnerable populations
• Sound clinical skills, including assessment, intervention, advocacy, psycho-education, supportive counseling, life skills coaching
• Sound group work and community work skills
• An understanding of the psychosocial rehabilitation model
• Knowledge of mental illness, medications, and ability to assist clients with symptom management
• Knowledge of community resources and systems
• Excellent communication skills
• Professionalism and strong team player
• Demonstrated organizational and problem solving skills
• Knowledge of Vietnamese culture and experience working with
Vietnamese people with mental illness
• Competence in relevant computer applications particularly Microsoft Office and Outlook
• Candidates with a proficiency in English and
Vietnamese are strongly encouraged to apply. Other language skill is considered as an additional asset
• A valid Canadian driver’s license and access to a vehicle is an asset
