Hong Fook Mental Health Association Vietnamese Social and Health Needs Assessment: Community Survey |
Hong Fook Mental Health Association offers a wide range of mental health services to the East and Southeast Asian communities including the Vietnamese community in the Greater Toronto Area. We request that you complete this survey; any information you can provide us will help us to better serve and understand the Vietnamese community.
In appreciation for completing this survey, your name will be entered into a draw for the following prizes: one 4.1 MP Sony Digital Camera, $50 gift certificates to major shopping centres, $10 gift certificates to movie theatres. Your identifying information will only be used for the prize draw and not for any other purposes.
Please return the survey no later than February 6th, 2006. We will contact the winners by phone and e-mail after February 13th, 2006.

Purpose
To
raise funds to support the multi-lingual, culturally appropriate
mental health services of Hong Fook Mental Health Association.
Theme
We have chosen the Elvis theme
because as Elvis had revolutionized pop music in his era, over the
past two decades, Hong Fook has pioneered a new approach to
community mental health and broken new grounds.
Our unique approach has not only enabled our clients to gain
better access to needed services, it has enhanced their chance of
integration into the community.
We are certainly hoping that, with continued support from the
community at large, we can sustain the same kind of success and
leadership in the community mental health field as Elvis had in
music. This is also the
year the music world is celebrating the 50th anniversary
of the release of Elvis’s first single which launched his
illustrious career.
