Case Management:

The Case Management Service works towards empowering Canadians of Cambodian, Chinese, Korean, Vietnamese and other Asian communities, who reside in the Greater Toronto Area, to attain ethno-racial equity in the mental health system and to achieve optimal mental health status.

Mental Health Workers facilitate the planning and coordination of services for the consumers and their family members. The worker assists the consumers and their family members with the goal of maximizing consumer empowerment and health through culture sensitive service, education and programs both within the Association and in conjunction with other service providers.


The major activities of Case Management include:

  • Consultation to individuals, professionals and agencies with regard to culture, case management, program planning and community resources.
  • Out-reach assessments or assisting in the assessment of individuals and families in their homes, agencies or institutions and providing recommendations to meet their needs.
  • Liaisons and referrals to link individuals and their families to appropriate resources.
  • Responsive and short-term intervention to individuals and families who are affected by various mental health issues/illnesses in different aspects of their lives.
  • On-going case management services (education, counselling, linking up with resources, skills training, etc.) for people with serious and persistent mental illness towards their road to recovery.
  • Various consumer groups aimed at improving the consumer's level of functioning and promoting independence in a supportive environment
  • Mutual support groups and educational workshops for family members of consumers to better cope with mental illness in their family.
  • Advocacy for individuals and their families for equal access to mental health services.
  • Linguistic and cultural interpretations to facilitate consumers to voice their needs.
  • Special English as a Second Language Class in collaboration with the Toronto Board of Education for people with mental illness who have difficulties integrating into regular E.S.L. classes.

Hong Fook is the only ethnocultural mental health case management service provider in Toronto specifically for the Asian population. Referrals and consultation to this Service come from a wide variety of sources including hospitals, social & mental health service providers, and settlement services agencies. Various service providers in different sectors are involved in rendering much needed services to people with mental illness. Some of the common needs include: psychiatric and other medical treatments; financial, housing; vocational; educational; legal; social; and recreational. The major service providers are: hospitals with in-patient and outpatient psychiatric services; community agencies with services focusing on one or more of the above mentioned need areas; and, government offices at different levels.


Who can call?

  1. Are 16 years of age and over with mental health problems; or a mental illness
  2. Live in Toronto, and
  3. Consent voluntarily to service
  4. Referrals are from individuals, families, friends, and health and social service providers
